Supported Living Coach

We are currently seeking a dedicated and compassionate Supported Living Coach, certified by the Agency for Persons with Disabilities (APD), to join our team. This role is crucial in supporting adults with developmental disabilities to live independently in their own homes and fully participate in their communities. The Supported Living Coach will assist individuals with learning the skills necessary to lead self-directed lives, manage their own homes, and maintain successful community inclusion — all in accordance with the principles of person-centered planning.

Primary Responsibilities

  • Provide individualized coaching to support clients in achieving independent living goals

  • Assist with skills training in areas such as budgeting, meal planning, housekeeping, hygiene, and social skills

  • Ensure the health, safety, and well-being of individuals in supported living settings

  • Advocate for individuals’ rights, preferences, and inclusion in community activities

  • Monitor and document progress toward life skills goals in alignment with support plans

  • Communicate regularly with support coordinators, families, and interdisciplinary team members

  • Assist with scheduling and attending medical appointments, coordinating transportation, and accessing community resources

  • Maintain all required documentation in accordance with APD standards and Medicaid waiver requirements

Minimum Qualifications

  • Certified Supported Living Coach through the Florida Agency for Persons with Disabilities (Required)

  • High school diploma or equivalent with at least 4 years of experience working with individuals with developmental disabilities, OR associate or bachelor's degree in a human services or related field with 1–2 years of relevant experience

  • Valid Florida driver’s license and reliable transportation (Required)

  • Level II Background Screening clearance (Required)

  • CPR/First Aid certification (or willingness to obtain prior to start)

  • Ability to work independently and maintain professional boundaries

  • Excellent communication and time management skills

Preferred Qualifications

  • Familiarity with APD’s Medicaid waiver documentation and iBudget requirements

  • Person-Centered Thinking training

  • Experience supporting individuals with behavioral or complex medical needs
